I, the person completing this form, have legal custody of the children named above and give consent for him/her to participate in events organized by Cornerstone Family and Kids Ministry of Cornerstone Church.  I understand that there are inherent risks involved in related activities, and I hereby release Cornerstone Church, its staff and volunteer workers from any and all liability for any injury, loss, or damage to personal property that may occur during the course of my child’s involvement. If my child becomes ill / injured, I consent to the staff and volunteer workers using their best judgment to give my child the over-the-counter medicine that they require and to use ambulatory and emergency services as necessary. I understand that they will attempt to contact me before providing the over-the-counter medicine, ambulatory, and emergency services but consent to them medically treating my child as necessary if they are unable to reach me.
